Get to know Sound Long-Term Care Management
The Sound Long-Term Care Management (SLTCM) story began in 2015 with Sound Physicians fostering value-based care partnerships in the acute care setting to improve care quality and outcomes. As a result of our efforts, Sound Physicians became one of the largest participants in Medicare's Bundled Payment for Care Improvement (BPCI) initiative, improving quality and lowering the cost for thousands of acute care patients.
Recognizing the opportunity to apply our proven approach to help even more at-risk Medicare beneficiaries, in 2023, we formed SLTCM as an accountable care organization (ACO) under the Medicare Shared Savings Program (MSSP).

Tom Kim, MD
Chief Medical Officer
Dr. Tom Kim is a board-certified internist who serves as Chief Medical Officer for Sound's accountable care organization, Sound Long-Term Care Management (SLTCM). A hospitalist by training, Dr. Kim frequently treated older, more vulnerable patients from skilled nursing and long-term care facilities. The hospitalizations for this patient population often brought unforeseen risks to their health and disruptions to their quality of life. As CMO for SLTCM, Dr. Kim has been instrumental in creating a clinical strategy that brings more resources to these at-risk, long-term care residents where they live, as well as to the providers who care for them. Dr. Kim also served on the value-based care team at Sound, leading to its success in the Bundled Payment for Care Improvement Advanced (BPCIA) program and other at-risk payer programs. His passion for caring for high-risk populations and commitment to being on the leading edge of healthcare led him to join the ACO team, where he was instrumental in establishing Sound's first Medicare Shared Savings Program. Dr. Kim trained at Virginia Commonwealth University and received his master's degree at Carnegie Mellon University.
Sandeep Yadavalli, MD
Associate Chief Medical Officer
Dr. Sandeep Yadavalli is a physician leader with over a decade of experience at Sound. Serving as Associate Chief Medical Officer of Sound Long-Term Care Management since 2023, Dr. Yadavalli is driven by providing exceptional care to his patients and contributing significantly to the broader health and wellness dialogue. For the past seven years, he's specialized in value-based care, with deep expertise in Medicare programs such as Bundled Payment for Care Improvement Advanced (BPCIA). He's been instrumental in growing Sound's portfolio of value-based arrangements to include Medicare Advantage plans and other risk-bearing provider entities. He is focused on care innovation and redesign, integrating technologies into care delivery, and stewarding change management. In 2019, he was honored with Sound's Core Value Award for Integrity. Dr. Yadavalli earned his medical degree from NTR University of Health Sciences in India and completed his internship and residency at Jefferson Einstein Philadelphia Hospital in Philadelphia, Pennsylvania. Prior to Sound, Dr. Yadavalli worked as a hospitalist at the Crozer Chester Medical Center in Upland, Pennsylvania.

Michael Camacho
President, Accountable Care and Telemedicine
Mike oversees Sound's telemedicine program and accountable care organization (ACO). Started in 2023, Sound Long-Term Care Management (SLTCM) is an enhanced-track ACO focused on long-term skilled nursing facility and assisted living patients. SLTCM currently has 12,000 patients in 43 states and is expected to grow to 30,000 patients in 2025, with Sound's telemedicine platform playing an instrumental role in improving patient care for SLTCM. Sound's second ACO, with 11,000 beneficiaries among 38 provider groups, will start in 2025 and continue the mission of providing best-in-class care to long-term and assisted-living residents. Mike has deep experience in the world of ACOs, having previously built and operated five highly successful Medicare Shared Savings Program ACOs along with several commercial payer ACOs over the last 15 years.
Carol Lyn Ayers
Chief Operations Officer
As Chief Operating Officer for Sound Long-Term Care Management and Telemedicine, Carol Lyn leads operations along with the client success and data analytics arms for SLTCM and Sound Physicians' telemedicine division. She works closely with her executive team to ensure that strategic initiatives are executed, partner needs are met, and strategic goals are exceeded to support the long-term growth and success of the organization. With more than 20 years in both the finance and healthcare sectors, Carol Lyn brings extensive experience in process design and improvement, project and portfolio management, and managing organizational change. Prior to joining Sound Physicians, she served as Chief of Staff to the Data Science division at Anthem, now Elevance Health, and established their program management office. She previously served as Vice President and Senior Change Consultant for Bank of America, where she led several multimillion-dollar initiatives focused on improvements in their home loan modification processes. Carol Lyn graduated summa cum laude with a bachelor's degree in business administration from West Liberty University in West Virginia.
