Leveraging over 20 years of experience in long-term and acute care, Sound Long-Term Care Management (SLTCM) has built a dedicated clinical team — including our Chief Medical Officer (CMO), Associate Chief Medical Officer (ACMO), and Quality Improvement Nurses — to support partner providers and facilities.

SLTCM offers access to the nation's leading LPTAC telemedicine platform to ensure patients receive timely medical attention when primary care teams are unavailable during evenings, weekends, and holidays. Subsidized for qualifying facilities, the platform connects nurses with Sound tele-clinicians, who — equipped with key patient insights from the EHR — provide expert guidance tailored to the needs of long-term care residents. This service adds an extra layer of support after hours for peace of mind and continual access to high-quality care.

Many long-term care patients live with chronic conditions that put them at risk for frequent hospitalizations. As many as 23 percent of discharged patients are readmitted within 30 days for the same condition. Our care coordination programs support patients, care teams, and caregivers in managing these conditions where they are — helping to prevent unnecessary hospitalizations and readmissions while improving continuity of care.
