Sound Accountable Care to Feature Top-Ranked ACO at LeadingAge National Conference This Month

Nashville, TENN. (Oct. 20, 2026) — Sound Accountable Care and Sound Virtual Care will highlight its leading long-term care focused ACO and telehealth service at LeadingAge’s annual convention, Oct. 25-28 in Philadelphia.

LeadingAge is a national association representing more than 5,300 nonprofit aging services providers advocating for high-quality care, housing and services for older adults. It’s annual meeting attracts innovators, policymakers, researchers and service providers to explore ways to build societies ready for longevity.

At 3 p.m. Tuesday, Oct. 27, Brian Carpenter, MD, SFHM, Chief Medical Officer, Telemedicine, will join a session titled “Primary Care Partnerships: How They Work, Why They Matter.” The session features two executives from National Church Residences: Angie Hoff, DNP, APRN, CNP, VP of Operations, Primary Care; and Margaret Gibson, PT, MPT, Vice President, Sales.

“We’re pleased to join National Church Residences for this timely discussion,” said Carpenter. “We’ll share how an ACO and a primary care group partnership can help senior living organizations strengthen resident health outcomes.”

In addition to co-presenting, Sound Accountable Care and Sound Virtual Care will exhibit at the conference, discussing how it became the nation’s No. 1 ACO in shared savings among MSSP ACOs focused on long-term care and facility-based beneficiaries.
